The 2026 Guide: How to Get Your Medical Cannabis Card in Kentucky

As of 2026, Kentucky’s medical cannabis program is now fully operational. More licensees will be coming online and more dispensaries will be opening each month. For many residents suffering from chronic conditions, this marks the beginning of a new chapter in healthcare options.
At the Kentucky Cannabis Industry Alliance (KCIA), our mission is to ensure safe, legal access for every eligible patient. Navigating state regulations can feel overwhelming, so we have broken down the entire process into four steps.
1. Check Your Eligibility
Kentucky law is specific about who qualifies for a medical card. You must be a permanent resident of Kentucky and have been diagnosed with at least one of the following qualifying medical conditions:
Chronic or Severe Pain
Post-Traumatic Stress Disorder (PTSD)
Cancer (any type or form)
Epilepsy or other intractable seizure disorders
Multiple Sclerosis (MS), muscle spasms, or spasticity
Chronic Nausea or Cyclical Vomiting Syndrome
Note: You must be at least 18 years old to apply on your own behalf. Minors may qualify with the assistance of a designated caregiver.
2. Find a Registered Medical Provider
This is the most critical step. You cannot simply visit a dispensary; you first need a written certification from a medical professional who is specifically registered with the Kentucky Medical Cannabis Program.
Many primary care doctors are not yet registered, so you may need to find a specialist.
3. The Evaluation
Once you select a provider, you will schedule an evaluation.
What to bring: Your Kentucky ID and relevant medical records (past diagnoses, medication history, etc.) supporting your condition.
The Visit: The provider will review your history and determine if medical cannabis is an appropriate therapy for you. Speak with your practitioner about cannabis products to ensure your prescription meets your needs, edibles, tinctures, flower etc.
The Certification: If approved, the doctor will enter your certification directly into the state’s electronic system. That is just one step - the patient must complete the following steps as well.
You must apply for a registry ID card within sixty (60) days of the written certification issuance date.
4. Complete Your State Registration
After your doctor certifies you, you will receive an email to complete your registration via the Kentucky Medical Cannabis Program Portal.
To apply for a card, an individual must:
Use the applicable registry identification card application form made available through the website for the Kentucky Medical Cannabis Program here.
Register for an account then access the Patient and Caregiver Registration Portal at https://ky-chfs-public.nls.egov.com/login
From the portal dashboard, select +Create New Application and Patient from the dropdown menu to open a New Patient Registration. If you are a caregiver, select Caregiver and New Caregiver Registration.
Registry identification card applications submitted by or on behalf of qualified patients must include:
The full name, address, telephone number, email address, date of birth, social security number, and driver's license number of the qualified patient;
A written certification issued by a medical cannabis practitioner;
The name, address, and telephone number of the qualified patient's medical cannabis practitioner;
Pay the application fee for a qualified patient ($25); and
A notarized signature page signed by the applicant that includes a statement by the qualified patient pledging not to divert medical cannabis to anyone who is not permitted to possess medical cannabis. Signature Page can be found Here

Approved cardholders will be issued a digital registry ID card within thirty (30) days.
Download, save, screenshot, and print a PDF copy by selecting Print Digital Card. Your card contains an individual patient number that will be validated before any purchase.
5. Visit a Licensed Dispensary
With your active card and valid ID, you can enter any licensed dispensary in the Commonwealth. Dispensaries are currently in the process of opening and more will be opening each month, until all 48 are operational around the Commonwealth. Not every county will have a dispensary

Frequently Asked Questions
Can I grow my own plants? No. Home cultivation remains illegal in Kentucky. You must purchase your medication from a licensed dispensary.
Is my card valid in other states? Some states offer "reciprocity," but you should check the specific laws of any state before you travel.
Will insurance cover the cost? Currently, medical cannabis evaluations and products are not covered by insurance.
